BA950 - Doctor of Philosophy (Business Management)

STEP 1:

1. Access the online application form: online.uitm.edu.my/ipsis/research/international/index.cfm
2. Key-in your Passport number to begin registration.
3. Press 'Enter'

STEP 2:

Please read the following before you proceed to fill the online application form
Fill the online application form

1. All the information must be accurate.
2. PART B, select option as given and make sure Campus is
   'B21-INSTITUTE OF BUSINESS EXCELLENCE UITM SHAH ALAM'
3. Required field ( * ) in the online form MUST NOT be left blank.

STEP 3:

Prepare the following documents:

1. Printed Online Application Form (CLICK HERE)
2. Photocopy of Passport - Front page (must be certified as true copy)**.
3. Photocopy of Bachelor Degree certificate - application for master program (must be certified as true copy)**.
4. Photocopy of Bachelor & Master Degree certificates - application for PhD program (must be certified as true copy)**.
5. Photocopy of Examination/Academic Tanscripts (must be certified as true copy)**.
6. Referee Reports – 2 referees (CLICK HERE to download the form)
7. Photocopy of English Proficiency Certificate. (CLICK HERE - English Proficiency Requirements)
8. Passport Size Photo (3 copies).
9. Application for Master or PhD by research must include a research proposal
10. Checklist for the required documents (CLICK HERE)

**  Photocopy documents must be certified as true copy by the authorize personnel/embassy/goverment.

All the documents stated above are COMPULSORY. Your application will be rejected if the required documents are incomplete.
